- Open a survey and go to Design view.
- Click laptop symbol.
- Select 'Background' on the left hand side.
- Edit background color or background image. (A good background image size is at least 1600 pixels wide and 1200 pixels high.)
- Save.

Set a nice background image and adjust the survey page opacity to show a bit of the background image.
