You can insert a Date picker -question to your survey where respondents can choose a date by clicking a calendar.
From question Settings you can define which dates are available to be chosen.

Future dates
Respondent can choose only dates that are in the future. Respondent cannot choose the same date they are giving the response on.
Past dates
Respondent can choose only dates that are in the past. Respondent can choose the same day they are giving the response on.
Within date range
You can define a range of days which are available to be chosen.
Excluded dates
None
All dates are available, which are defined in Allowed dates.
Custom dates
You can define specific dates which cannot be chosen
Weekends
Respondent can only choose dates from Monday to Friday
Weekdays
Respondent can only choose Saturday or Sunday
First day of week
Default
Default day is based on answer language. For example, if answer language is English, default day is Sunday.
